Requesting a New I-20 or DS-2019

Please present your current visa document (I-20 or DS-2019), I-94 card, passport and all other supporting documentation when requesting a new visa document.

Request Form

NOTE: as of August, 2002, the Form IAP-66 was replaced by the Form DS-2019.

To return to the United States after travel abroad,
you generally need the following documents:

  1. A passport that is valid beyond the date of your intended reentry into the U.S.
  2. A visa stamp that will be valid at the time you reenter the U.S.
  3. A valid I-20 or DS-2019 with an ending date later than the date you plan to reenter the U.S. (If you will need to apply for a new visa stamp to reenter the U.S., you should request a new visa document.)

PLEASE consult our section on "Travel Abroad and Reentry" for additional information at http://www.internationaloffice.duke.edu/Main_Travel_Border.html

PLEASE check to see if you need a visitor's visa to enter other countries by contacting the appropriate embassy in Washington, D.C. or by consulting an embassy's web site (Search for Visa Services).

Students on F-1 Optional Practical Training: you will need a valid I-20 and a valid I-766 (Employment Authorization Document or EAD) to reenter the U.S.

Is your visa stamp still valid?

  1. If you have a valid visa stamp in your passport, check the issue date of your I-20/DS-2019. You will find the date to the right of the signature of the person who issued your document. If you plan to reenter the U.S. more than 1 year AFTER your document's issue date, then you need to request that a new document be issued for travel purposes or that your current document be revalidated for travel. We strongly urge you to request a new visa document if you will be conducting any business at an embassy or consulate, most especially if applying for a new visa stamp. If your visa stamp still is valid, you will not have to provide updated financial funding documentation. Again, if you plan to travel outside and reenter the U.S. during your program, make sure that your Duke-sponsored I-20 or DS-2019 is not more than one year old from the original issue date.
  2. If your visa stamp has expired, then you need a new visa document and you must provide updated financial funding documentation (bank statement, departmental support letter, etc.) and submit it with this form. A letter from your academic/program advisor confirming you are currently enrolled and meeting all program requirements will be necessary, and please refer to the example below. (You also may want to direct your advisor to our web site to see this sample letter.)
